PHASE 4 REHABILITATION SERVICES
Why Clinical Rehabilitation Differs from Regular Training
Our exercise rehabilitation is always guided by the clinical requirements of your diagnosis, medical history, and current activity levels — not by trends or preferences. All sessions are led by Phase 4 specialist trainers with extensive qualifications and experience across a broad spectrum of conditions.
“Exercise is medicine” — it remains the single most evidence‑based lifestyle intervention for the prevention, management, and rehabilitation of chronic conditions.
Safety Through Continuous Monitoring
Your safety is our highest priority. For cardiac patients, heart rate is carefully monitored throughout each session, with readings taken at two‑minute intervals to ensure training remains within medically safe zones.
For those managing diabetes, blood glucose levels are assessed before and after each session. Exercise intensity is strategically managed to optimise fat utilisation over glucose, supporting both safety and effectiveness.
Your Rehabilitation Journey
1. Initial Consultation – A relaxed but professional meeting where we discuss your diagnosis, treatment history, and recovery goals. Wherever you are in your journey, we work with you to design a bespoke plan that optimises recovery while maintaining the highest safety standards.
2. Tailored One‑to‑One Sessions – All sessions are individually led by the most appropriate Phase 4 trainer for your condition, ensuring highly specific and relevant programming. Frequency is determined at the outset to achieve maximum benefit and efficiency.
Online Consultations
We understand that travel, particularly in or around London, can be challenging during illness or recovery. Our online consultations offer the same expertise and personal attention as an in‑person session — enabling you to receive professional care in the comfort of your home.
Through secure virtual platforms, we conduct detailed assessments, discuss your needs, and deliver guidance tailored precisely to your situation. For many clients, this flexibility has allowed them to maintain consistent care without interruption.
Process
Stage 1 – Introductions
We begin with a brief introduction, allowing you to understand who I am, my professional background, and the scope of the services I provide. At this stage, we will arrange an initial consultation and reserve a suitable date in the diary.
Stage 2 – Consultation
This is our opportunity to connect on a deeper level and establish the foundation of our work together. During the consultation, I will outline the process in detail, address any questions or concerns, and gain a clear understanding of your objectives. We will explore your goals, expectations, motivation, timelines, medical history, fitness background, lifestyle, nutritional habits, and any barriers to exercise. In person, we can also identify specific considerations that may require tailored strategies before beginning your programme.
Stage 3 – Post-Consultation
Following our meeting, I will follow up to address any further questions and provide essential documentation for your review, understanding, and signature:
• Informed Consent
• Client Rehabilitation Agreement
• Physical Activity Readiness Questionnaire
These documents must be completed in full before commencing any form of physical training.
Stage 4 – Programme Setup & Resources
Once all preliminary steps are complete, you will receive access to tailored digital tools and resources to support your journey both inside and outside of our sessions:
• MyFitnessPal – to track and manage your nutritional intake.
• PT Hub – to organise, monitor, and review your training programme remotely.
These platforms enhance accountability, sustain motivation, and ensure you have professional support at your fingertips—wherever you are.
